​As the Joseon king falls gravely ill, a desperate attempt is made to save him using a mysterious herb called "saengsacho." However, the king is resurrected as a zombie, and the royal family works to keep this a secret. Crown Prince Lee Chang seeks to uncover the truth behind his father's condition and the mystery of saengsacho but is ensnared in a web of royal conspiracy, forcing him to flee. ​"The term 'life and death' used in the drama is derived from the original Kingdom, specifically from 'Shinui Nara' (Land of the Gods). It seems to be inspired by the 'life and death disease' that zombies are said to suffer from in the original, and the official name for the zombies in the drama was also decided to be 'life and death disease.' The setting takes place before the Byeongjahoran (Manchu invasions), in the period following the Imjin War (Japanese invasions of Korea) and the Jeongyu Jaeran (second Japanese invasions). However, it is not a precise reflection of these historical events. Instead, the drama is set in a chaotic post-war era, inspired by the aftermath of the Imjin War."​
